Copies may be available for a minimal fee from the Records Department which is open Monday-Friday, 8:00 am - 4:00 pm. A signed request for records form is requested but not mandatory and is available on the Records Department page of this website. Each request for records must be evaluated for availability. Copy and research fees may be applicable.
